
Cost Worksheet Instructions
This worksheet will allow you to calculate the estimated cost of different surveying and reporting options you are considering.
All orders will include some quantity of questionnaires and at least one of each of the reports that RMBSI creates from that type of survey. Pricing for the questionnaires varies depending on both the type of survey you are ordering and the quantity you use. To view a chart showing the pricing of the different survey types at various quantities, click on the large red "Pricing" button at the top of the screen on the right side. Costs for reports remain the same regardless of how many you order. In some cases you may need to order just one report and in other cases you may need several different reports. To use this worksheet, type in the quantity for each type of questionnaire you want to order, and the quantity for each type of report you want to order.
For example, suppose you want to survey 1500 students at two separate high schools. You want to know how much the order would cost if you have us prepare just one district-wide report (combining the data from the two schools) versus if you order a separate report on each school’s results (2 reports). You also want to know how much it would cost if you use the Adolescent ADAS by itself versus if you use the ADAS with the Prevention Planning Survey.
To check the cost on the first option, type in "1500" in the quantity box for the Adolescent ADAS forms, and type in "1" in the quantity box for the Detailed Adolescent ADAS Report. Then click the "Calculate Total" button. The proper price per form will fill in automatically, and the total will display in the "Total Estimated Cost of Order" box at the bottom of the page. If you’d like, you can print this information by clicking "File" "Print" on your menu bar.
Then, to find out how much your next possible order would cost, click the "Reset" button. All the fields will be cleared. Now you can type in different quantities or check the cost of different surveys and reports, and view the new "Total Estimated Cost of Order" by again clicking the "Calculate Total" button. We suggest that you print the form each time you calculate a different variation so that you have a record of how each of your options would alter your budget.
